Changing passports may entail changing the name and/or the gender marker.

The exact process in which to change the name or gender marker on your U.S. passport, depends on when it was issued.

If your passport was issued over 15 years ago you must apply for a new passport.

[edit] Applying for a new passport

To apply for a new passport, you must present in person:

  • Form DS-11, "Application for a U.S. Passport", [1]
  • Two recent passport-quality photographs (2" x 2")
  • A valid ID
  • A birth certificate
  • A certified copy of your name change court decree (when changing name)
  • A notarized affidavit from your surgeon that a sex change operation has been completed (when changing gender marker)
  • A fee of $100 (for people over 16)

[edit] Modifying and renewing a passport less than one year old

When renewing a passport less than one year old, you must mail off:

  • Form DS-5505, "Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement", [2]
  • Your current passport
  • Two recent passport-quality photographs (2" x 2")
  • A certified copy of your name change court decree (when changing name)
  • A notarized affidavit from your surgeon that a sex change operation has been completed (when changing gender marker)
  • (No fee is required)

[edit] Modifying and renewing a passport over one year old

When renewing a passport over one year old, you must mail off:

  • Form DS-82, "Application for a U.S. Passport by Mail", [3]
  • Your current passport
  • Two recent passport-quality photographs (2" x 2")
  • A certified copy of your name change court decree (when changing name)
  • A notarized affidavit from your surgeon that a sex change operation has been completed (when changing gender marker)
  • A fee of $75 (for people over 16)
